COVID-19 Assumption of Risk, Release, and Waiver Liability Form

The World Organization has declared the novel Coronavirus (COVID-19) a worldwide pandemic. High Praise Ministries the Destiny Center has undertaken reasonable steps to lessen the risk of transmission of COVID-19. Please check the boxes below. Please sign, date, print name and the names of your children under 18 years old, who will be attending service with you.

I have not experienced symptoms that of fever, fatigue, difficulty in breathing, dry cough, or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.

I have not been, nor any member(s) of my household, diagnosed to be infected of COVID-19 virus within the last 30 days.

I am fully aware that participation in the services carries with it certain inherent risks related to COVID-19

I hereby voluntarily accept and assume all risk of loss, personal injury, sickness, death, damage, and expense arising from such inherent risks.

I being 18 years of age or older, do hereby forever release, waive, discharge and covenant not to sue High Praise Ministries the Destiny Center and itâ€™s past, current and future officers, directors, employees, members, volunteers, contractors, representatives from any and all damages, injuries, losses, liability, claims, causes of action, litigation, or demands, including but not limited to those for personal injury, sickness, or death, as well as property damages and expenses, of any nature whatsoever which may incurred, directly or indirectly, now or in the future, in any way related to COVID-19.

This COVID-19 Assumption of Risk, Release and Waiver of Liability Agreement shall be binding to my heirs, executors, administrators, successors and assigns.

I have read and understood this Agreement and enter into it voluntarily in consideration of the opportunity to participate in the church services. I acknowledge I am giving up legal rights and/or remedies which may be available to me
